Wireless controllers remember the last device they were paired with. When they are used with a phone or PC, then brought back to the console — or when the battery runs very low — they often lose their pairing and need to be connected again.
Almost every controller can be re-paired with a USB cable in seconds, which is always the first thing to try.
Try these first — 30 seconds each
[ ]Charge the controller for at least 30 minutes — very flat batteries cause failed pairing.
[ ]Turn off Bluetooth on nearby phones and laptops the controller has been paired with.
[ ]Use a data-capable USB cable; some cheap cables are charge-only.

Re-pair with a USB cable
PlayStation 5 and PlayStation 4: connect the controller to the console with a USB cable and press the PS button. Xbox Series X|S and Xbox One: connect with USB and press the Xbox button. Nintendo Switch Pro Controller: connect to the dock with a USB cable. The controller pairs automatically.

Pair wirelessly from scratch
Xbox: press the pair button on the console, then hold the small pair button on top of the controller until the Xbox logo flashes quickly. Switch: go to Controllers → Change Grip/Order and hold the sync button on the controller. PC: open Bluetooth settings, put the controller in pairing mode (hold Share + PS on a DualSense, or the pair button on an Xbox controller), and add it.

Reset the controller
PlayStation controllers have a tiny reset hole on the back; press it with a paperclip for about five seconds, then re-pair with USB. Xbox controllers can be reset by holding the Xbox button for 10 seconds to turn off, then turning back on.

Remove old pairings
On a PC or phone, remove the controller from the Bluetooth device list so it does not reconnect there instead. On consoles, delete stale controllers from the Bluetooth or accessories menu.

Update the controller firmware
Consoles update controllers when connected by USB — check for a prompt. On PC, the Xbox Accessories app updates Xbox controllers, and Sony's firmware updater tool updates DualSense controllers.

Test with another console or cable
If the controller will not connect even with a cable on a different console or PC, the port or controller may be faulty. If other controllers do not work on your console either, the console's wireless may need attention.
Platform notes
PC
Steam's controller settings (Settings → Controller) can conflict with other controller software. If buttons are mapped strangely, check Steam Input options for that game.
Switch
Joy-Con controllers pair by sliding them onto the console. If one side disconnects often, check for a system update and keep the console out of cabinets that block signal.
Stop it happening again
Keep controllers charged and avoid storing them completely flat for months.
Keep a data-capable USB cable near the console.
Unpair controllers from PCs and phones when you are done using them there.
When to get professional help
If the controller does not charge, has sticky or drifting sticks, or will not pair with any device, check the manufacturer's warranty and repair options; stick drift is a common hardware issue that is often covered.
